General Information
Certificate Request (.pdf, 23K)
Undergraduate and Post-Baccalaureate Certificates are available. Students planning to earn a certificate must complete all course work at Capitol College with a cumulative GPA of 2.0 (Undergraduate) or 3.0 (Graduate) in all required certificate courses. There is no limit on the number of certificates earned. Each certificate costs $25.00. Students are given an official certificate, as well as recognition on their official transcript.

Use this form to update your contact information. This includes for personal and professional contacts.
INDEPENDENT STUDY FORM (.pdf, 16K)

Independent study is not taught.  You are responsible for learning material ‘independently.’  The professor will proctor tests, grade assignments and submit a grade at the end of the semester.  Please complete the student section, have the professor complete their section then forward or send to the Office of Registration and Records.  You will still have to complete a registration form for the class.

Transfer Credit Approval (.pdf, 32K)
Please complete this transfer credit approval form before taking any courses at another institution of higher learning.  This will assure the coursework is transferable to your degree program.  Once the course is completed with a grade of 'C' or better, have official transcripts sent to Capitol College attention Registrar's Office.
Prerequisite Waiver Registration Form (.pdf, 37K)
This form must be completed and approved in order to waive a prerequisite.
Transcript Requests

Transcripts will be issued when you submit a signed request form (if your financial account is current). The cost is $10.00 for each copy. Separate forms must be submitted for each request with a different address.  Please note that transcript requests made during grade collection may be delayed until grades are processed.
Release of Records Forms
Verification Form (.pdf, 17K)
This form is used to generate an enrollment verification letter from the Office of Registration and Records.  Verification of attendance is often required by student loan originators and health/automobile insurance companies.  Please allow two to three days for processing.
Consent to Release Records Form (.pdf, 421K)
If your finances are handled by a third party (sponsor, employer or parent), you may be required to provide a grade report.  This form gives the Office of Registration and Records permission to mail your grade report to the appropriate party.

Degree Change
Undergraduate Change in Degree Form (.pdf, 32K)
To add, drop, or change a degree program, complete the form. The form must be signed by the Academic Dean or Department Chair before it will be processed by the Office of Registration and Records.

Diploma Replacement
If you need to reorder your diploma, complete the Diploma Replacement Form and submit it to the Office of Registration and Records. The fee is $75.00. Please allow 4-6 weeks for processing.

Registration
Register for Classes
After viewing the course schedule and consulting with your advisor (if need be), complete this registration form.  It will automatically be submitted via email to the registrar's office for processing.
Add/Drop Form
Courses may be added once the semester has begun until the beginning of the second week for term classes and the beginning of the third week for semester classes. (Refer to the academic calendar for exact dates each semester.)Courses may be dropped or changed to audit status (from credit status) until the beginning of the fourth week for term classes and the beginning of the eleventh week for semester classes. (Refer to the academic calendar for exact dates each semester.)
Withdraw Form
If you want to withdraw from the college or are dropping all classes in a term or semester you must complete this form. You are not permitted to withdraw during the week of final exams. As long as you are in good standing and are not out for over a year, you may register for classes without reapplying to the college.
